Documents Upload - Minors

All copies of Driver License and Insurance cards must be front and back, no exceptions.

 

Required documents for Psychological testing, Neuropsychological evaluations, and Counseling:

    • If parents are legally married:
      • Driver license (or ID card) of the parent accompanying the child
      • Primary subscriber's Driver License
      • Insurance card/s (primary insurance and, if the case, secondary or supplemental insurance)
    • If parents are divorced:
      • Driver License (or ID) of the parent accompanying the child
      • Driver License (or ID) of the parent with whom the child lives
      • Copy of any court/divorce paper (if available)
      • Both parents need to sign, individually, the Intake Packet form
      • Insurance card/s (primary insurance and, if the case, secondary or supplemental insurance)
    • If parents are separated:
      • Driver license (or ID card) of the parent accompanying the child
      • Primary subscriber's Driver License
      • Insurance card/s (primary insurance and, if the case, secondary or supplemental insurance)

 Previous Evaluations/Assessments 

    • Entire copy of any previous evaluation, including medical reports, school assessments, psychological evaluations. 

Physician's Referral for testing or counseling

    • Referral slip

For copy of any Medical record (including CT-Scan, MRI, EEG, PET-Scan and X-Rays)

    • Brain scans: please provide us a copy of the CD you received from your physician, at least 2 days before the starting of the evaluation.
