What are your business hours?
We are open 7 days a week, from Monday to Sunday, morning, afternoon and evening (excluding State and Federal holidays).

What is your earliest available time?
We have openings throughout the week.
Do you offer emergency services?
We do not offer emergency services. If this is a life threatening situation or an emergency, please do not hesitate to contact the 911 or to go to the nearest Emergency Room (ER).
What forms of payment do you accept?
We accept only credit cards and debit cards. If you do not have a valid credit card you will need to pay with a cashier check.
What insurance do you accept?
We take most HMO, PPO and EPO from all major insurance plans, including but not limited to the following carriers:

- Beacon Health Options (Value Options)

- Blue Cross Blue Shield
- Blue Shield of California – Medi-Cal
- Cal Optima

If you recently moved from another state or don’t see your insurance listed above please give us a call and we will check directly with your insurance carrier.
I don’t have a health insurance, how much will it cost?
Please follow this link as guidelines, however keep in mind that every person is different and your cost maybe lower than what is indicated on our website. After we meet for an initial consultation we will provide you with an estimate of the cost associated with the treatment you need, so you can make an educated choice based on your available resources.
Depending on your needs a psychological evaluation ranges from $450 to $4000 or more.
When do I pay my co-payment or my deductibles?
Your co-payment is expected at the time we meet. Once your credit card is added to your chart you are billed for your share of the cost (ex. co-payment). your insurance is billed for the remaining part of the balance.
Upon request we can provide a billing statement usually at the end of your last scheduled session.
How do i schedule an appointment?
If you are interested in receiving treatment at our office, please call us at (626) 354-6440 or fill out our online form to make an appointment for an intake (if you want to see the doctor the same day please call us before 2:00pm).
What if I have to cancel my appointment?
Unforeseen events can happen, such as illnesses, unfortunately your insurance does not reimburses, nor covers cancelled or skipped sessions.
You can call us and reschedule your appointment with at least 3 (three) business days before your appointment date.
All cancellations received with less then 3 (three) business days advance notice will incur the following fees:
- Psychological tests: $320
- Neuropsychological tests: $425
If there is an emergency and you are hospitalized you will be charged the above fees, however once you present us the document showing your visit to the ER or the UCC a credit shall be issued for that charge.
Do you offer free consultation?
We do not offer free consultation.
Is there a waiting list?
No, as of January 2018 we do not have a waiting list.
what if I need medications?
We do not prescribe medications, but we can refer you to a wide network of physicians who can evaluate your needs.
What is an intake and how long is it?
An intake is your first appointment, a time for you to meet with our doctor to determine how we can team up with you to address your needs. We will explore your concerns, what tests you need (or what have been requested by your physician), your schedule, what documents to bring to your next appointment, what is your financial responsibility (ex. co-payment) and review together any pertinent medical history.
This first appointment, commonly known as consultation, goes from 45 minutes to 2 hours, depending on your needs.
Please arrive 25-30 minutes before your appointment time to complete all the required paperwork (if needed), unless you have already filled out the Webform for psychological tests or the online form for neuropsychological assessment, and received a confirmation email with your intake packet.
If you have filled out the Webform or the online form and you have not received your confirmation email, check your spam folder or call our office at (626) 354-6440.
If receive treatment at your office, can anyone find out about it?
NO. The content is strictly confidential.
However you can, at any time, authorize the release of your records according to federal laws and our office’s policies. This means that if you contact us by phone, by fax or email and asks us to release your records to a third party we may not be able to proceed without first authorizing us in written.
If you want someone else to be informed that you are receiving, or have received, services at the PMMT (ex. your lawyer), you must sign a specific authorization to disclose information. The PMMT may keep paper copies of your file or electronic records.
Information about your contact with the Counseling Center will not be shared with others outside the Counseling Center without your written consent, except in specific circumstances described below:
- You are, or your dependent child is, a danger to yourself or others;
- The information involves the abuse or neglect of a child, developmentally disabled person, or dependent adult. Therapists are required to report such incidents to authorities;
- You bring charges against your therapist or the counseling center;
- You are obtaining therapy to avoid jail or commit a crime;
- You are using pedo-pornographic material, including downloading from the internet;
- We receive a court order for your records.
There are other cases not mentioned above where we are required to provide your medical records to the requesting party, if this is a concern for you asks your therapist how you can protect your privacy or consult with your attorney before disclosing any information in therapy.
